Free implant load log (spore test and release record)
This printable log is for dental offices in the United States that place implantable items in their sterilizer loads, and it is free to use and print.
An implant load log records every sterilizer load that contains implantable items on one row: what the load held, its spore test and result, and the date the items were released.
This one is a free sheet for US dental offices: type into it before you print, or print it blank and write it in by hand; nothing you type is stored or sent.
What the sources say
Each line below restates one source and ends with its name. Where a source hedges, the hedge stays.
-
Use a biological indicator for every sterilizer load that contains an implantable device, and verify the results before using the implantable device, whenever possible.(CDC, 2003 dental infection-control guidelines)
-
Use biological indicators for every load containing implantable items and quarantine items, whenever possible, until the biological indicator is negative.(CDC, disinfection and sterilization guideline, 2008)
-
A biologic indicator (spore test) is used at least weekly and with every load containing implantable items.(CDC, Infection Prevention Checklist for Dental Settings)
-
Implantable items are quarantined until spore test results are known; in an emergency, placing implantable items in quarantine until spore tests are known to be negative might be impossible.(CDC, 2003 dental infection-control guidelines)
-
Ideally, implantable items should not be used until they test negative.(CDC, best practices for sterilization monitoring in dental settings, 2024)After a positive spore test, recall and quarantine any implantable items and do not use them until they are shown to be sterile.(CDC, best practices for sterilization monitoring in dental settings, 2024)
-
Do not sterilize implantable devices unwrapped.(CDC, 2003 dental infection-control guidelines)
The printable log
One row for every load with implantable items. Fill it in on screen and press Print, or print it blank and write it in by hand. Nothing you type on this page is stored or sent anywhere.
Why the last columns exist: the sources word the hold differently — implantable items are quarantined until the results are known “whenever possible” (CDC, 2003 dental infection-control guidelines; CDC, disinfection and sterilization guideline, 2008), and “Ideally” they are not used until they test negative (CDC, best practices for sterilization monitoring in dental settings, 2024); in an emergency the quarantine might be impossible (CDC, 2003 dental infection-control guidelines).
A good log has a “Released before result?” column with room for the reason, so a release before the result stays visible on the row.
Prints on one US Letter page, landscape.

One row for every load with implantable items. In the “Released before result?” column, write yes or no, and when it is yes, write the reason beside it. This sheet holds no patient names.
How to use it
Four steps, from a blank page to a signed record of each implant load.
Fill in the header
Office name, the sterilizer’s label, and the month. One sheet per sterilizer, per month.
One row per implant load
Write the date, the sterilizer, the load number, and the implant items the load held.
Add the spore test
Write the BI lot and the time it was placed. When you read it, write the result (negative or positive) and the date.
