Free dental sterilizer register (every unit, one sheet)
One printable sheet for a US dental office to list every sterilizer in the office: its label, type, manufacturer, model, serial number, cycle types, and when it was installed or qualified. The sheet is free to use and print, and nothing you type on this page is stored or sent anywhere.
What the sources say
Points from CDC's disinfection and sterilization guidance for dental settings. Each line ends with its source.
-
Steam, ethylene oxide and other low-temperature sterilizers are tested with biological and chemical indicators upon installation, when the sterilizer is relocated or redesigned, after major repair, and after a sterilization failure has occurred.(CDC, 2008 disinfection and sterilization guideline)
-
Each type of steam cycle used for sterilization (e.g., vacuum-assisted, gravity) is tested separately, and in a pre-vacuum steam sterilizer three consecutive empty cycles are also run with a Bowie-Dick test.(CDC, 2008 disinfection and sterilization guideline)
-
Pre-vacuum sterilizers should be tested periodically for adequate air removal, as recommended by the manufacturer.(CDC, 2003 dental infection-control guidelines)
An air-removal test (Bowie-Dick test) must be performed daily in an empty dynamic-air-removal sterilizer.(CDC, 2008 disinfection and sterilization guideline)
These two sources do not agree on how often. This sheet records the test for each unit and leaves the schedule to the office.
-
Routine maintenance for sterilization equipment is performed according to manufacturer instructions and documented by written maintenance records.(CDC, infection prevention checklist for dental settings)
-
Place the date of sterilization, and if multiple sterilizers are used in the facility, the sterilizer used, on the outside of the packaging material.(CDC, 2003 dental infection-control guidelines)
That label is why this register asks for each unit's label as it is printed on packs.
The printable log
One sheet lists every unit in the office. Type on screen before printing, or print it blank and write it in by hand.
Prints on one US Letter page, landscape.
Sterilizer register
One sheet for every unit
| Label (as printed on packs) | Type (steam pre-vacuum, steam gravity, dry heat, chemical vapor) | Manufacturer | Model | Serial no. | Cycle types (target temperature and time) | Installed / qualified on | Air-removal test applies (pre-vacuum only) | Notes |
|---|---|---|---|---|---|---|---|---|
Scroll the table sideways to reach every column.
How to use it
Four steps, from a blank page to a register of every unit.
Fill in the header
Office name, the sterilizer label and the month at the top of the sheet.
One row per unit
Each sterilizer gets its own row: the label as it is printed on packs, then the type, the manufacturer, the model and the serial number.
Cycles and dates
List each cycle type with its target temperature and time, and write the date the unit was installed or qualified on.
Air-removal and notes
Mark the air-removal column for pre-vacuum units and leave it blank for the others. Use notes for anything else worth keeping with the unit.
